Physical Education:

Sample Worksheets > Physical Education Worksheets.  This site was one of the only sites that I was able to find physical education worksheets.  The samples given seem usable and very appropriate.  However the downfall to this site is that it is a members’ only site, with it costing $29.99 to become a member.

Physical Education.  This site is Michigan’s Department of Education’s web site.  This site is useful to physical educators by providing worksheets that include fact sheets and also assessment tools.

NASPE – National Association for Sport and Physical Education.  This is a great resource for physical educators.  While it does not have specific worksheets to test students, it can provide information and even pamphlets on information regarding physical education and physical activity.

Health:

Worksheet Generator Worksheets to GO.  This website offers health worksheets and the worksheets are listed by category.  It is easy to navigate and offers simple matching activities or word scrambles.

Worksheets 4 Teachers: Health Work Sheets and Printables.  This web site contains worksheets that relate to health information.  The information in this site is also arranged by categories.  The worksheets are brief and easy supplements to a lesson.

Health Theme Unit.  This site is by far the best relating specifically to health education.  It could be easily used by a teacher for supplemental learning material for the subject of health.

Science (Anatomy):

Human Body Worksheets.  This site is the best site I have come across regarding anatomy worksheets.  It offers a large variety of worksheets.  The site is easy to navigate and use.

Awesome Library – Material_Search.  I could not find any heath or physical education worksheets at this site.  However in the science section I found worksheets pertaining to the anatomy of the eye and ear along with a worksheet on the brain and nervous system.

Science Lesson Plans, Worksheets, Teaching Lessons, Activities.  This science page contains information that could be used in an anatomy class.  Information includes worksheets on the human body specifically an overview of the body, and the digestive, circulatory, nervous, and respiratory systems.  This page is a members’ only page but I did not find a fee, just a sign up page.

Free Science Worksheets from the Kid’s Page Archive.  This page contains many science worksheets for the K-5 level.  Two that could be utilized as a supplement to a health or PE course would be the worksheet/crossword called “Your Body”.  It is a lesson on anatomy.
