After reviewing a wide variety of elementary math lesson plan sites, I have found the following to be the most interesting and easy to use:

 

NCTM: Illuminations Lessons

Backed by the National Council of Teachers of Mathematics, this site provides a wide variety of lesson plans, sorted by NCTM standards and grade level.

These lesson plans are broken down by grade and most include a list of vocabulary words to go along with the lesson.
