I have found a number of websites that have worksheets related to library skills. This list is divided into 4 sections. There are general research skills worksheets, graphic organizers, book genre worksheets, and worksheets on the Dewey Decimal System. These worksheets are appropriate for middle school students.
General Research Skills Worksheets
This website features various forms and worksheets for
research reports. The forms feature
various subjects for which students could do a short research assignment. You will find forms that are appropriate for

like math, science, and language arts.

This worksheet helps students understand the various
sections of the library through several different activities. Students must find book titles and call
numbers from different non-fiction subjects, they must
put call numbers in order, and must list numbers and categories for different
